Every driver operating a vehicle under your commercial fleet policy needs to be assessed and recorded before they get behind the wheel. This driver declaration form lets you formally register a new or additional driver against your Truck Insurance HQ policy, giving your insurer the information needed to assess risk accurately and keep your cover valid.

Information Required

Please complete every section as thoroughly as possible. We need the driver’s personal details, licence information, and a short history covering convictions, licence suspensions, traffic offences, prior insurance issues, medical conditions that could affect driving, and any at-fault accidents or claims in the last five years. Answering honestly is essential, since undisclosed information can affect a claim later, so it is worth taking a few extra minutes to get it right the first time.

If you have answered yes to any of the questions above, it helps to attach supporting documents, such as a driving history report from your state transport department or details of any criminal convictions. You can upload these directly using the file upload field further down the form.
